The Catacombs
You enter a room with two passages and an archer on a cliff. One of the passages is blocked by a boulder, so you will be forced up the other passage to the two grates, step on the floor plate to open the grate ahead and you'll be on the cliff. Walk along the edge to you right and crouch to enter the cave tunnel (Image 1). You will end up inside the catacombs behind the inaccessible grate. From this point on, smash every crypt you find.
The Golden Chalice
After you enter the catacombs, turn into the first passage to the left (Image 1), where you will come to a door on your right and some steps leading down. Take the door on your right and go through the room back down the steps to a drop-off into the lower catacombs (Image 2). Read the sign about where untold riches lay. Jump across for the Stone of Summoning (ducking during this jump helps some classes), then jump down into the lower catacombs and walk forward, taking a right into a dead end, Push the red button on the lower left corner of the empty grave to open it and find the Chalice (Images 3 & 4).
The Altar
Come out of the empty grave and go straight to the grave in the wall at the end of the room (Image 1). Break it open and walk through, going up the ramps into the church. Place the Chalice on the Altar (Image 2), releasing the boulder that blocked the passage at the start, and allowing it to roll through the center of the church, so don't get smushed. The boulder will smash open a new passage, allowing access to a chain mechanism.
The Mask of King Lazarus
Throw the chain mechanism's switch (Image 1), raising the bars blocking the two doors. Go through either door and down the stairs to a large room. On either side of the room are button in the wall depressions, one button hidden behind a statue you must smash (Image 2). Press both buttons to activate each half of the door in the center alcove at the room's front, exposing a closet that holds The Mask (Image 3). When you grab the Mask, a stairway will open in the floor (Image 4), giving access to The Hall of the Dead.
