Hexen started as an extension of Heretic. Ironically, the game was described initially as Heretic II. However, when the team figured out how many major changes had been made (such as multiple character classes; the hub system; all new weapons, items, and monsters; etc.), the name was changed and it was turned into a completely new game. Amazingly enough, Hexen was completed in about 8 months. The biggest legacy for Raven games from Hexen was the hub system, which was used in some form or another in several subsequent games. Depending on whether you like that kind of thing or not, this was either a very good or very bad development.
